Part 1a Physiotherapist (a) Copy of Certificate of Registration / Practising Certificate (Both are acceptable)
Part 1b Physiotherapist (a) Copy of Certificate of Registration / Practising Certificate (Both are acceptable) (b) Official letter of verification from employer(s) documenting a minimum of 2,000 clinical hours (applicants can use the template HKPA provided or company's own letter. The letter should include applicant's full English name, working period and hours, must be signed and chopped by employer, and include the employer's contact information.) (c) Self-declaration form of physiotherapist's clinical experience hours (Part 1b) (d) Declaration before a Commissioner for Oaths, Notary Public, or Justice of the Peace for the two documents (Document (b): Employer's letter + Document (c): Self-declaration)
