What is the CPD Record Form and when is it due?
It is the official record the Hong Kong Physiotherapists Board requires every registrant to complete and keep at the end of each 3-year CPD cycle, listing all CPD activities for audit by the Education Committee. The current cycle runs 1 July 2023 – 30 June 2026.
Is generating the Record Form free?
Completely free. Both the CPD Record Form and the Self Study (Category IV) form are free-forever tools — there is no paywall.
Are my name and signature stored?
No. The name, PT registration number and signature you enter are used only in your own browser to generate the PDF — nothing is uploaded to or stored on our servers.
